Frequently Asked Questions : Employee Access & Disclosure

May I take pictures of a patient?

Employees should never take unauthorized pictures of patients or other sensitive information, even with their own personal cell phones. Many cell phones are capable of easily sharing pictures and videos with others, including uploading such material to publicly available websites. Even if the material is not deliberately shared, the cell phone or its internal memory card can easily be lost leading to a potential disclosure of protected health information.

In cases where photos of a patient are required for use in a publication or for other purposes, it is necessary to complete the Photo Authorization Form in conjunction with Attachment 46, the 3rd Party Authorization Form .
